Getting to Know You — Online Form

HELLO & WELCOME! If you'd like to secure a session date and begin the communication process, complete this online form. All questions are required, but if you're unsure of how to answer, or the question doesn't apply, simply type "unsure" or "N/A."

At the bottom of the form are instructions for session payment. Please note that payment, in-full, is due on session day (unless prior arrangements have been made) and a $100. non-refundable deposit is due at session booking.

Depending on your collection choice, you'll have one wardrobe change or more. We'll be talking about wardrobe together. Once determined, please lie clothes, shoes, accessories etc. out and text me a quick pic with your phone ;) *Important- please have clothing wrinkle-free at your session
